Pursuant to Subsection 18-357(i) of the City of Peoria Municipal Code, the following,

hereby requests that no peddlers as defined by the City of Peoria Municipal Code, be allowed to peddle in the above listed subdivison, Peoria, Illinois, and affirm the following:

The majority of the duly elected officers of the Homeowners Association voted for approval and authorized the below listed officer to submit the "No Peddlers Allowed" Request Form on behalf of the homeowners association.

I, The undersigned, affirms that the information contained in this "No Peddlers Allowed" Request Form is true and accurate to the best of my knowledge and belief, and that upon or near the main entrance, or entrances, of the residential subdivision, a weatherproof sign which is plainly readable and states the following: "NO PEDDLERS ALLOWED IN THIS SUBDIVISION" or words to the same effect will be posted at the expense of this Homeowners Association or Subdvision.
